Testosterone therapy doesn't seem to increase the risk of prostate cancer, but it can stimulate prostate cancer cell growth. Because prostate cancer is so common, doctors tend to be wary of prescribing testosterone to men who may have a higher-than-average risk of having undiagnosed prostate cancer. If the patient does not respond or cannot tolerate CPAP, testosterone must be reduced or discontinued.

Testosterone replacement therapy, in the form of injections, pills, patches, or gels, can improve the signs and symptoms of low testosterone levels in these men. Hypogonadism is a clinical condition in which low serum testosterone levels are found in association with specific signs and symptoms. People who are assigned female sex at birth produce testosterone, but in lower amounts and have higher levels of estrogen. Despite this controversy, testosterone supplementation in the United States has increased considerably in recent years.

However, in men with Alzheimer's disease, testosterone treatment appeared to improve quality of life without affecting cognitive measures. Testosterone deficiency is a common disorder in middle-aged and older men, but it is underdiagnosed and often goes untreated. Psychotic symptoms, excessive libido and aggression, as well as physical and psychological dependence and withdrawal syndromes have been reported rarely during testosterone treatment (306, 307), although the validity of testosterone or the cause are uncertain. In fact, a study involving 2,295 adolescent men and boys found that lack of sleep may be related to lower testosterone levels (3).

In the presence of a clinical picture of testosterone deficiency and borderline serum testosterone levels, an attempt could be made to conduct a brief therapeutic trial.
